Decision:

The Pathway to Care Grant Request was approved.

Reasons for the Decision:

To provide funding to in order to build a partition wall to divide the bedroom, to meet the needs of the young person placed in their care.

The home extension has allowed the young person to be 

discharged from care by enabling them to permanently live

with people within the family and friends’ network of the

young person.
