Meeting: 12/04/2018 - Health Select Commission (Item 86)

South Yorkshire, Derbyshire, Nottinghamshire and Wakefield Joint Health Overview and Scrutiny Committee Update

Minutes:

The Scrutiny Officer reported that the Committee had not met since the last update.

 

The report on the outcome of the Hospitals Review was due to be finalised towards the end of the month and would be submitted to the Select Commission during the new Municipal Year as well as an update on Stroke Care and Children’s Care and Anaesthesia Services.


Meeting: 01/03/2018 - Health Select Commission (Item 77)

Joint Health Overview and Scrutiny Committee for the Commissioners Working Together Programme

Minutes:

The Health Select Commission received an update report from the Scrutiny Officer concerning the Joint Health Overview and Scrutiny Committee (JHOSC) for the Commissioners Working Together Programme.  The issues highlighted from the recent meeting held on 29th January, 2018 were:-

 

(a) the Terms of Reference had been refreshed and the name of the committee amended to be the South Yorkshire, Derbyshire, Nottinghamshire and Wakefield Joint Health Scrutiny Committee; questions from the public would also now be a standard agenda item at each meeting.

 

(b) as new NHS work streams and potential service reconfigurations emerged, the JHOSC would determine whether it was appropriate for the committee jointly to scrutinise the proposals under development; this would be based on the scope, time-frames and geographical footprint that could be affected by potential changes; each local authority reserved the right to consider issues at a local level.

 

(c) Implementation plans were progressing on the changes to children’s surgery and anaesthesia - detailed work to agree the clinical pathways through the Managed Clinical Network and a series of designation visits to the hospitals;  the expected implementation was in quarter one of 2018-19. 

 

(d) The JHOSC also received a re-cap of the information that had informed the decision on Hyper Acute Stroke Units; an update on the Hospital Services Review; and a request from NHS partners that the JHOSC would convene to scrutinise the Hospital Services Review.

Meeting: 30/11/2017 - Health Select Commission (Item 51)

Minutes:

 

The Health Select Commission received a short verbal update from the Scrutiny Officer. 

 

Hyper acute stroke

The Joint Committee of Clinical Commissioning Groups (JCCCG) met on 15th November 2017 to consider the business case and make a decision on the proposals for hyper acute stroke services.  The executive summary of the business case, link to the full business case and powerpoint presentation to the JCCCG meeting had been included in the Members’ information pack.

 

The unanimous decision was to support the proposed option to cease providing hyper acute stroke services at Barnsley and Rotherham hospitals.  There would be a phased implementation to ensure patient safety and to ensure that the changes were manageable for the hospitals.  Implementation would be closely monitored by the JCCCG and by the JHOSC.  The service would be decommissioned in Rotherham from July 2018 and in Barnsley by January 2019 with hyper acute stroke services provided in Sheffield, Doncaster, Chesterfield and Wakefield.  The new model required approximately £1.8m investment for tariffs and patient transport and the pathway would include thrombectomy.

 

Hospital services review

The purpose of the review was to explore how services could be delivered to ensure local people had access to safe, high quality care provided by the most appropriate healthcare professionals and in the best place.  The key was future proofing and sustainability of services.  It was very important to reiterate that the review was not looking at closing any of the current general hospitals in South Yorkshire, Bassetlaw or Chesterfield. 

 

The five services in scope were:

 

·         Urgent and Emergency Care

·         Maternity

·         Gastroenterology including endoscopy

·         Stroke care - early supported discharge and rehabilitation

·         Hospital services for children who are particularly ill

 

Consultation had commenced in the summer and there would be a public event on 6th December 2017 at The Source, Meadowhall.  There would also be other opportunities for local people to get involved, including an event for Elected Members in January 2018.

 

JHOSC

The next meeting would take place on 11th December 2017 and the agenda would include progress on implementing the changes in children’s surgery and anaesthesia agreed earlier in the year; an update following the decision on hyper acute stroke care; and an update on the Hospital Services Review. 

 

The agenda would be published on 1st December 2017 and HSC members were asked to submit any questions to the Chair by

7th December.

The Health Select Commission received a short verbal update report from the Scrutiny Officer concerning the Joint Health Overview and Scrutiny Committee (JHOSC) for the Commissioners Working Together Programme. 

 

Hyper Acute Stroke

 

The Joint Committee of Clinical Commissioning Groups had all the information required to inform the business case and would be taking a decision at the next meeting in November 2017.

 

Hospital Services Review

 

Further to the papers circulated at the last Commission meeting, the Hospital Services Review would produce its final report for the end of April 2018.  The review had been structured in two stages:

 

·           Stage 1 Assessment (June – December 2017).  This stage included an assessment of the sustainability of services across the South Yorkshire and Bassetlaw footprint to agree a shortlist to be taken forward for a more detailed assessment of sustainability issues, and identification of the problems with these services.

 

·           Stage 2. Options and New Models (January 2018 – April 2018).  This stage would focus on potential solutions to the issues identified.

 

The final list of services in scope was not yet available.  A detailed project plan had been developed, including governance groups and communications.  Consultation had commenced with patients and the public.  Any further information would be circulated to keep HSC fully briefed.

The Chair gave an update from the last meeting of the Joint Health Overview and Scrutiny Committee (JHOSC) held on 31st July, 2017:-

 

Children’s Surgery and Anaesthesia - plans for implementation would be in place by the end of December 2017, with a further update to the JHOSC, probably in October. 

 

Hyper Acute Stroke – the decision from the Joint Committee of CCGs was due in the autumn with an update expected for the next JHOSC.

 

South Yorkshire and Bassetlaw Hospital Services Review – a new workstream under NHS transformation had commenced recently:-

 

- to define the criteria to help understand what a sustainable hospital
  service would be. 

- to look at services and define those which were non-sustainable.

- to advise on future models of delivery to ensure long term sustainability.

 

JHOSC Terms of Reference – a refresh was under way and would be communicated to the Select Commission. 

 

Copies of a powerpoint presentation about the hospital services review and a stakeholder briefing were circulated to Select Commission Members at the meeting.

Meeting: 15/06/2017 - Health Select Commission (Item 8)

Joint Health Overview and Scrutiny Committee for the Commissioners Working Together Programme

Minutes:

The Health Select Commission received an update from the Scrutiny Officer concerning the Joint Health Overview and Scrutiny Committee (JHOSC) for the Commissioners Working Together Programme.  The issues highlighted:-

 

-          The decision on the reconfiguration proposals for Hyper Acute Stroke and Children's Surgery and Anaesthesia had been postponed from May until 28th June.  However, the Joint Committee of Clinical Commissioning Groups would only be making the decision on the Children's Surgery and Anaesthesia on that date as there was further work taking place with regard to Hyper Acute Stroke.  The new date had not been announced for that decision.

 

-          There would be another meeting of the JHOSC in July.  This would provide an opportunity to discuss the final decision for Children’s Surgery and Anaesthesia and to discuss future scrutiny following any changes.

Minutes:

The Commission’s consultation response for both Service proposals had been submitted online on 10th February with only minor amendments from the version circulated for comment.

 

The JHOSC would meet on 3rd April with an additional CSC meeting on 29th March to consider the papers as they would include the final outcomes from the consultation and the high level business case.

 

The Joint Committee of Clinical Commission Groups was to meet on 18th April to make the final decision.


Meeting: 19/01/2017 - Health Select Commission (Item 73)

Joint Health Overview and Scrutiny Committee for the Commissioners Working Together Programme

Minutes:

Janet Spurling, Scrutiny Officer, reported that NHS England had carried out their planned mid-point review of the public consultation in late December led by the Consultation Institute.  The review had found that, although the Christmas period had been taken into account regarding the length of the consultation period, it had had a greater impact than planned as the response rate was lower than expected given the approach and methodology used.

 

The consultation deadline had been extended until 14th February to allow for further engagement.  This meant that the final decision would now be made by the Joint Committee of CCGs in April, 2017.

 

In light of the concerns expressed by the Select Commission regarding the proposals for Hyper Acute Stroke Services, the Chair and Vice-Chair had met with a representative of the South Yorkshire Ambulance Service to discuss their ability to deliver the service, the number of vehicles they had at their disposal currently and how many more they felt they would need to carry out the new service.

 

The representative had been quite clear that the Service had no problems in maintaining the service whatsoever nor were there any issues with regard to the skilling of staff.  The only issue was that the Service felt it needed an additional three ambulances bearing in mind the new proposed models of working.

 

Members reiterated the importance of having clear data and evidence when officers and partners were presenting information to the Commission.

 

Resolved:-  (1)  That the report be noted.

 

(2)  That the Commission submit a collective response to the consultation.


Meeting: 01/12/2016 - Health Select Commission (Item 60)

Joint Health Overview and Scrutiny Committee for the Commissioners Working Together Programme

Minutes:

The following verbal report was given on the above Programme:-

 

Consultation

-          900 hits on the website and interest via Twitter but these were not being converted into consultation responses as yet even though the information was getting out to the public

-          As at 21st November there had been:-

 78 responses on the Hyper Acute Stroke proposals with 46 disagreeing with the proposals

60 responses on Children’s with 30 disagreeing with the proposals

-          Very low attendance at public meetings with no-one attending the 18th November meeting at MyPlace in Rotherham or the meeting at the Source in Sheffield the following week

-          NHS England were now looking at a gap analysis across all the communities and engagement so far to ensure they were reaching into communities and welcomed any suggestions from Members

-          There had been feedback from all areas on both Services, Hyper Acute Stroke and Children’s, but mainly from Barnsley (49 Stroke/26 Children)

 

Ambulance Service

-          East Midlands – already had the specialist centre model in place for Stroke Care, Coronary Care and major Trauma and were achieving better outcomes and reduced mortality

-          Yorkshire Ambulance Service Staff Training – all frontline staff (Paramedics and Technicians, call handlers for 999 and 111 as well as Community First Responders), were taught to assess the patient suspected of Stroke using the FASt.  Patients at point of call had a fast assessment which was repeated at the time of the face-to-face assessment.  If it was a suspected Stroke staff followed the Yorkshire Stroke Pathway and referred the patient to the nearest Hyper Acute Stroke Unit

 

Children

-          Data to come on the number affected by the proposals on the 6 sub-specialities

 

The Chairman and Vice-Chairman would continue to be involved, feeding in Members’ issues and concerns and reporting back from the JHOSC.


Meeting: 27/10/2016 - Health Select Commission (Item 47)

Joint Health Overview and Scrutiny Committee for the Commissioners Working Together Programme

Minutes:

Janet Spurling, Scrutiny Officer, reported the following:-

 

-          Consultation had now commenced on the proposed changes to the Hyper Acute Stroke Care and non-specialised Children’s Surgery and Anaesthesia

 

-          The final consultation documents had reflected some but not all of the feedback from the Joint Committee and Health Select Commission

 

-          A Frequently Asked Questions document had been produced which answered some of the concerns and questions raised

 

-          The Rotherham Foundation Trust needed to do things differently to be sustainable and had realised a few years ago the need for collaboration even as a standalone Trust.

 

·           Proposed model for Stroke Care reflected that for Coronary Care which was a recognised as a good model.  Manchester and London also had a centralised model of Hyper Acute Care

·           No Rotherham patients would go to Chesterfield for Hyper Acute Stroke Care

·           Children and young people would go to the nearest hospital to where they lived

·           Discussions with staff would take place if changes took place and, due to shortages of skilled staff, the NHS would be looking to match expertise across the region to provide the services

·           Planning and managing bed capacity for the extra numbers of patients in the proposed 3 hospitals were currently being discussed

 

The next meeting of the JHOSC was to be held on 21st November when there would be an update on how the consultation was progressing and the business cases for change.  The Yorkshire Ambulance Service were to be invited to discuss the issues raised with them. 

 

The Chairman would feed back at the next Health Select Commission.
